If a Handicapped Parking Permit has been lost, mutilated or stolen, a duplicate permit may be obtained by completing only the top portion of the application and submitting the application to the City Clerk or designated County official with proof of identification. The duplicate permit will be mailed to the applicant by the Department of Motor Vehicles.

The expiration date remains the same and the medical portion does not need to be completed. When a duplicate permit is issued, all permits issued prior to issuance of the duplicate become void. If a person locates their original permit after a duplicate is issued, the original permit may be destroyed.
